Summary:     Hydro_RNSID: optionally depend on MPI for parallel HDF5

in case Cactus uses parallel HDF5 one needs to provide the include PATH
etc for mpi.h for hdf5.h's use.

Summary:     Hydro_RNSID: make utils compile even if the thorn is moved or renamed

this requires some make trickery to extract the arrangement and thorn
name

also fixes some leftover "CarpetIOHDF5" references in the makefile as
well as making sure that CFLAGS is used and not CXXFLAGS.
