#286: Writing/reading grid structure should be explicit operations

Carpet should offer explicit routines to write and read all grid structure
metadata. One method could be to serialise this information to a string
that can then either be written to HDF5 (as is done now), or could be
 interpreted by post-processing scripts.

This would clean up checkpointing, and would also allow reading/writing
arbitrary grid functions at a later time.
