 hmm, then I have no idea what causes it (and am unable to reproduce it
 with for example the output_buffers.par file in CarpetIOHDF5/par). I can
 reproduce an out-of-range exception when I try to output grid arrays, eg.
 SphericalSurface::sf_radius. I fixed the bug responsible for that and
 pushed the change.
