 I did some initial debugging on this (hampered by my unfamiliarity with
 CarpetLib's internals), and so far the extra piece of information is only
 that the failed function is a {{{CarpetLib::restrict_3d_vc_rf2<double, 0,
 1, 1>}}} called (with some intermediaries) from reflux_all where both
 source and destination boxes have (integer) coordinates that look cell
 centred (though that might be ok).
