 I have merged the latest master with the CCZ4 branch, included Erik's
 changes, created regression tests based on the existing BSSN tests and
 added sample parameter files for gauge waves, robust stability test and a
 binary black hole. The existing testsuites still pass. OK to merge back
 into master?
