[Carpetx-developers] index tag in CarpetX retired, use centering instead

Announcement: I have removed support for defining grid function centering via index tags for grid functions. This did not work correctly any more (since the flesh does not look at these tags), and there is now a better way via centering declarations. Please update your thorns. CarpetX will otherwise produce a run-time error.
