#2554: init_3_timelevels produces nans on past timelevels of the initial slice

I am actually not sure if this ever worked as advertised. The code in question is:
```c++
initialise_3tl_flip_timelevels(cctkGH);
initialise_3tl_evolve(cctkGH);
initialise_3tl_evolve(cctkGH);
// TODO: May want to restrict where possible (i.e. if the time
// refinement factor is one)
initialise_3tl_recycle(cctkGH);
```
which is bad because `initialise_3tl_evolve` steps all reflevel by one step, so that in step 1 when say reflevel 1 wants to do prolongation for the buffer zones there are only 2 valid timelevels on reflevel 0 \(namley tl=0 and tl=1\) and thus interpolation in time picks up nan from tl=2.
A fix is proposed in: [https://bitbucket.org/eschnett/carpet/commits/7147f842569c37719d1c97d5f6ddd… essentially moving the reflevel loop outside of `initialise_3tl_evolve`. Still has issues with the restriction involved but I don’t think those can be avoided.

Gallery example is updated.
Run was successful yesterday. Created the images and checked the VisIT singularity command, all good. Updated the web page; they were minor changes. Also used the “git config pull.rebase false” for the merge strategy, then the “git pull” then I could “git push” \(after “git add \[the new files\]” and “git commit -m \[text\]” \).
Also uploaded the tarball. Created the Bitbucket APP Password using my Vanderbilt SSO login. Still had to do the curl from my laptop. Did not work from our computing cluster. Curl command below, and cut and paste the APP Password from dumb text editor when asked.

Gabriele, Roland: I am afraid there isn’t an easy fix to this, without breaking some of the key use cases. If one is not interested in those cases, the offending lines can be simply commented out. The correct way to solve the problem would be to introduce a mechanism to register grid functions for restriction when necessary \(moving the burden of ensuring correctness to the user\). This has been on the thorn’s todo for a while, but has not been implemented yet.
