Yes, in principle they may be caused by wrong parameters, in particular concerning number of boundary points. However, there may also be an error in determining the grid point weights, e.g. when the grid shapes are very weird (concave corners, grids too close to each other, etc.)

It appears only after a limited (small) number of regriddings (close to one another) during the inspiral, not at the very beginning of the simulation.
